Downloadable Forms

The following ITS forms are now online in Acrobat (PDF) format. You will need the free Adobe Acrobat Reader in order to download and print these forms (see the Software Download page if you do not have this application).

Please return completed forms in person with your College ID to the ITS Main Office (Muller 102). Please allow 3 business days for processing.

Student & Organization Accounts
This form is used for:

  • New Web Server Accounts (for managing an Org. Web site)
  • New E-mail Accounts
  • New/Renew/Take Ownership of student organization e-mail accounts
  • New/Renew/Take Ownership of student organization Web sites
 
Listserve (MajorDomo) Account Request Form
This form is to be used to request new, renew and assume responsibility for Majordomo lists. If you want to announce your list to all student, the Mailing List Announcement Request Form must also be completed.
 

Mailing List Announcement Request From
This form is to be used by registered IC student organizations who wish to announce their oganization's Majordomo (listserv) list to all students. This can only be done once per semester.
