Cayuga Health at Ithaca College is pleased to welcome you to Ithaca College!

Prior to coming to Ithaca College you must provide us with information about your health and immunization history. This is required by New York State law. It also serves to protect your health and that of your fellow students, and to provide us with information that will help us serve your health needs while you are here at Ithaca College. All health care records at Cayuga Health at Ithaca College are confidential and are completely separate from any other College records.

Please fully complete all steps outlined below.

STEP 1: Immunization Record

Obtain a complete immunization record from your health care provider or school. Only copies of official records, such as school immunization records or verified records from a health care provider will be accepted. You will be required to enter your immunization dates and upload a legible picture or scanned copy of this immunization record on the Medical Clearances tab.

It is not required that you use our Immunization Form, but if you need one to take to your provider this one is available for your use.

STEP 2: College Credential Activation 

Set up your IC account if you haven’t done so already (via IC Connect). You will need your IC username and password in order to access the MyICHealthCenter web portal.

STEP 3: Required Forms

Log in to the MyICHealthCenter web portal using your IC username and password. Select the “Medical Clearances” tab from the menu at left. You will need to update all of the “Items Required for Clearance” and submit them electronically.  These include: 

  1. HEALTH HISTORY FORM: This is where you will provide us information about your personal and family health history to include current medications and allergies. 
  2. IMMUNIZATION RECORD: This is where you will upload the complete immunization record you obtained in STEP 1. 
  3. MEASLES/MUMPS/RUBELLA: This is where you will enter your MMR or individual measles, mumps, and rubella vaccinations and dates.   
  4. MENINGITIS RESPONSE FORM: Only complete this form if you are declining the meningitis vaccine. If you have received the vaccine in the last 5 years and provided proof, you do NOT need to complete this form.
  5. TB SCREEN-RISK FORM: If you answer “yes” to any of the questions on this form, you will need upload the results of recent tuberculin testing or a chest x-ray.  

STEP 4: Minor Forms (under 18)

This step is required ONLY IF you will be under 18 years old as of June 1st. You will see three additional requirements on the Medical Clearances tab.  These are: 

  1. MINOR CONSENT TO TREAT 
  2. MINOR MENINGOCOCCAL FORM 
  3. MINOR NPP (Notice of Privacy Practices) 

You must have the Required Forms for Minors completed by your parent or guardian and upload the signed forms on the appropriate tab above. 

STEP 5: Verbal Communication Release Form

Log in to the MyICHealthCenter web portal using your IC username and password. Select the “Medical Clearances” tab from the menu at left. Please download and print this document. After the document is completed, upload it to your MyICHealthCenter portal under medical clearances, verbal release form update. You can also download here at IC Health Certification Forms with IC ROI.pdf (ithaca.edu). This form does require a witness signature. This can be a parent or guardian or an academic mentor. 

STEP 6: Additional Items

 Please utilize your immunization record from STEP 1 above to update all applicable immunizations in the “Additional Items NOT Required for Clearance” list. This will enable us to maintain a complete health and immunization record that you will have access to for future use.

You can update profile, upload both front and back insurance card under insurance tab on IChealthportal.

Submission time frames

Please complete these requirements by June 12st for fall admission or one week before the beginning of classes for spring or summer admission.  

We will notify you via secure messaging to your Ithaca College email account if anything is incomplete. It is your responsibility to open and read all secure email messages sent from Student Health Services.   Failure to submit the requirements as outlined above will result in you not being allowed on Ithaca College property.  You will be unable to attend classes, remain in residential housing, or use IC facilities.
