STEP 1. Please Look at the https://www.ithaca.edu/file-download/download/public/94760 to determine if we participate with your insurance.
STEP 2. Please contact the customer service number on your insurance card to confirm if you are in or out-out-network. You will refer to us as "Ithaca Medical Services PLLC". Our TAX ID number is 86-2943718 and NPI number is 1164166559.
STEP 3. Please confirm your deductible and copays.
Any amount not paid by your insurance is your responsibility.

* July 10th, 2025: Enrollment and Waiver Process Open
* August 10, 2025: First Day of Coverage for Students Enrolled in SHIP for 2024-2025
* September 15, 2025: Deadline to Submit Waiver Form to Decline SHIP Coverage.
