All faculty, staff, and students seeking to drive College vehicles owned, leased, or rented (outside rental agency) must submit a Driver Information Form and be deemed "acceptable" before operating College or rental agency vehicles; in addition, students must also complete a driver safety course.

The Off-Campus Film / Media Production and On-Campus Film / Media Production forms will need to be completed for any media film/media productions off-campus and on-campus. Once the Dean's Office completes their review they will contact Risk Management to request the Certificate of Insurance (COI).

• A COI will be issued only when the entity specifically requests one or it is required by a contract/agreement. Be sure the Student Film Shoot Request Form contains the complete name, address, and method to receive the COI.

The Medical Information Form can be used by faculty and staff who are coordinating student events or trips as a way to ensure that important medical information is on hand in the event of an emergency. The forms must be retained in a secure location in the relevant department/school for 3 years from the date of the event/trip for all participants who are age 18 and older and must be retained indefinitely for minors (those under age 18).

Each person participating in an activity must sign a Release Waiver and the event organizer must sign the waiver as a witness.  Waivers must be retained in a secure location in the relevant department/school for 3 years from the date of the event for all participants who are age 18 and older and must be retained indefinitely for minors (those under age 18).

Be sure to select the appropriate waiver ages 18+ or minor and note #1 on both waivers must include the specific name, location, date (including year), and any alternative/rain dates for the activity. In addition, you should review the potential risks and injuries in #2 and include additional items applicable to your activity, as necessary.
